Photographs: Submit one full image and one detail image. Please include a full image of the back of Wearable Art as well.

Digital images should be formatted at 300 dpi or higher. Image size should be a maximum of 6" on the longest side (1800 pixels). JPEG or TIFF image formats acceptable.

Label each image with last name and first initial in ALL CAPITAL LETTERS, then the title of your entry, then the word “Full” or “Detail.”

Entry Fee: $15.00 per Entry (to be paid on 2nd part of form)

Optional: Become an Individual Member of the La Conner Quilt & Textile Museum for only an additional $35.00.

Terms and Conditions:

A check in the box below indicates your acceptance of the following terms and conditions in regards to La Conner Quilt & Textile Museum’s 2012 Quilt Festival:

I understand and agree that:

1. my Entry Fee is non-refundable and must be included with this Entry;
2. if my Entry is juried into the 2012 Quilt Festival, I am committed to having my Entry displayed for the duration of the Festival;
3. I am responsible for the costs of shipping and insuring my Entry to and from the La Conner Quilt & Textile Museum (upon arrival all entries will be covered by the Museum's insurance policy);
4. photographs submitted with my Entry are true and accurate depictions of my Entry, they have been taken using the Museum's photography specifications, and I agree to allow the Museum to use the images of my Entry in any and all advertising, promotions and publications, be they digital or in print;
5. if my Entry wins First Place in its Category, or is a Bonus Award Winner, it will be honored by inclusion in a group exhibit in the Museum from October 10 through December 30, 2012;
6. if my Entry is unavailable for exhibition during this time period, arrangements may be negotiated with the Museum Curator for future exhibit;
7. my Entry has not been exhibited in the Museum's Quilt Festival in the past.
